Jola Forys QN

Jola is a Children's Community Matron from Walsall Community Health NHS Trust. Her role is to act as a case manager for children and their families ensuring that they receive the best possible care and preventing hospital admission wherever possible by supporting them at home. She provides training and ongoing clinical support to carers of children who are technology dependent. She is happy to mentor other QN applicants.
Tracy Hall QN

Tracy is a Community Matron and joint clinical champion for COPD. She works for NHS Stoke on Trent. As a community matron she works with individuals to help manage their long term conditions. She is passionate about district nursing. She has also served as a PEC nurse, and as joint clinical champion she is able to improve on care provision for people with COPD. She is happy to help mentor other QN applicants.
Julie-Anne Hughes
Julie-Anne is a District Nurse Team Leader for a conjoined team covering two GP practices within South Staffordshire PCT. There are nine members of the team. She has responsibility for the bank nursing service, recruitment and induction. She is part of the Strategic End of Life Group and she is a Gold Standards Framework Co-ordinator. She is an Independent Nurse Prescriber adn has has an Advanced Physical Assesements qualification. She is happy to mentor other applicants for the QN title.
Jayne Richards QN

Jayne is a Community Matron with Shropshire County PCT. She has been a district nurse in Yorkshire and Shropshire, a leg ulcer specialist and an NVQ assessor. Now, as a community matron, she works with adults with multiple long term conditions. The role is to encourage self management of conditions through education to improve quality of life. Along with a colleague, she has been responsible for developing this service in Shropshire. She is happy to mentor other QN applicants.
Jenny Tarver QN
Jenny is an Immediate Care Nurse working for Shropshire County PCT. Jenny says, "I work within the immediate care team the main aim role is to prevent unavoidable hospital admission and facilitating early discharge. I have over 20years community experience and qualified as a Specialist Practitioner in 2001. I am currently studying for my MSc in Advanced Clinical Practice and a special interest in managing Respiratory conditions."
