Young people with long term health conditions face particular challenges when they move from children’s to adult health services.

The QNI has carried out a project to improve the experience of young people navigating this process, with a specific focus on district nursing and general practice nursing.

This project involves:

  • identifying evidence-based approaches to improving the transition experience of young people
  • developing resources and ‘transition champions’ to promote learning and embed best practice.

Literature Review

To inform the work, the QNI has undertaken a literature search to identify relevant research. The search comprised UK literature published within the last ten years.

An article about the project was published in the British Journal of Community Nursing (30 December 2016).

Online Learning Resource

We have also created a new online learning resource for nurses, to help them support young people through the transition process.

The project work was led by Queen’s Nurse Candice Pellett OBE and funded by The Burdett Trust for Nursing.
