Discharge Planning: Best Practice in Transitions of Care – Hard Copy

This report provides a summary of work undertaken by The QNI on behalf of the Department of Health to identify the barriers and challenges preventing effective discharge from hospital to home.

The report seeks to provide key recommendations that contribute to an improved discharge experience for patients, carers and their families, and identifies examples of effective discharge planning. The report includes the findings from the literature review, explanation of the methods used to collect data, examples of best practice and recommendations following the analysis of data.
